Job Title: Community Navigator

Job purpose: Assist agency clients, who are primarily individuals and families facing challenging life circumstances in need to mental health services, to access community based care and resources that promote wellness and recovery.

Job Duties: The Community Navigator will
• Conduct initial and on-going needs assessments with clients
• Assist clients in developing engagement plans, based on the results of the needs assessment
• Provide motivation for clients, especially those who are discouraged or do not have the drive to achieve their recovery goals
• Provide information about and referrals to mental health providers and substance abuse treatment programs
• Provide support in attending mental health therapy sessions and substance abuse treatment
• Assist clients in getting and coordinating medical care, including finding a medical home
• Provide and coordinate transportation for activities and appointments that maintain and support health and wellness
• Assist clients in applying for housing, employment, and educational opportunities
• Provide connections to soup kitchens, food banks, and other community food programs
• Engage with the criminal justice system, including law enforcement officers, the Public Defender’s Office, the Commonwealth Attorney, and the Courts
for clients who may have committed non-violent offenses to develop alternatives to incarceration
• Provide support and assistance on issues pertaining to probation/parole, especially related to charges stemming from the results of mental illness
and/or substance use
• Provide support and advocacy as appropriate for issues pertaining to separation/divorce and child custody
• Offer other support that might be indicated to promote wellness, recovery, and more complete community integration
• Collect/compile client demographic data and employee time-use data per the direction of the Executive Director
• Represent and promote the MHA: C-A Community Navigator program at community meetings at the direction of the Executive Director

• Bachelor’s degree in a human-services related field preferred, but willing to consider relevant experience in lieu of formal education
• Experience working with adults experiencing challenging life circumstances (mental illness, substance use disorders, homelessness, recent
incarceration, etc.)
• Ability to incorporate the principles of trauma informed care into service provision
• Knowledge of available mental health and substance use disorder treatment services in the Charlottesville area
• Knowledge of local food, housing, employment, and legal-aid resources in the Charlottesville area
• Valid Virginia driver’s license, current car insurance, and own reliable transportation
• Ability to use Microsoft Office Suite applications
• Ability to work collaboratively and with minimal direct supervision
• Excellent interpersonal skills
• Ability to quickly build rapport and communicate effectively with diverse populations (clients, clinicians, law enforcement officers, etc.)

Work Schedule: This is a 32-40 hour per week position. The ideal candidate is willing to maintain a flexible schedule (possibly working some evening and/or weekend hours) in order to meet the needs of our clients.
